** Each participant can submit only one completed/drawn poster.
** Size of the poster will be 1’6” X 1’ (1ft 6 inch by 1 ft).
** Participants will fill up this form and submit it with their posters in Human Milk Bank Office, ground floor, Neonatology building, IPGME&R, SSKM Hospital within 26th July, 2016, 4pm.
** Mention the name of the participant in the poster.
** Submitted posters will be not returnable and can be used in promotional works of this institute.
** Names of the winners will be declared on 3rd August, 2016 at LT-5, Academic Building, IPGME&R – SSKM Hospital. Participants are requested to be present in that programme from 2pm.
** Photocopy of this form can be used.
