RSHAA invites application for Mukya Mantri Chiranjivi Swasthya Bima Yojana Logo Design Competition 2021

Eligibility

The Contest is open For All Rajasthan. Registered Organizations under relevant Indian laws are also eligible to participate.

Contest Period

The contest will remain open from 7 April to 15 April 2021 5 PM.

How to Participate?

All entries must be submitted to the website health.rajathan.gov.in/MMCSBY. Entries submitted through any other medium/ mode will not be considered for evaluation.

Logo Submission Steps

STEP 1 : Go to URL health.rajasthan.gov.in/mmcsby and click on the link given For Mukhya Mantri Chiranjeevi Swasthya Bima Yojana Kindly Click on Below Link for Registration https://health.rajasthan.gov.in/content/raj/medical/en/home.html

STEP 2: Click on the Logo Submission link

STEP 3 : Click on Proceed

STEP 4 : Fill the details Name, Mobile No., Em ail ID, District and Address.

STEP 5 : Then Upload Government ID proof, Click on choose -> Select file and click on Upload.
STEP 6 : Then Upload Logo in JPG/PNG format, Click on choose -> Select file and click on Upload.

STEP 7 : Click on Submit and note down the Transaction ID generated on screen. HURRAY!! Logo Submission Successful

Note :
** Uploading Government ID proof is mandatory. Gov t ID proof can be Driving License, Voter ID, Aadhar Card, PAN card, Passport or any government issued valid card.

** Only single entry can be submitted from on e mobile number.
** File size should not be more than 5 MB (max).

Technical Parameters

** Participants should upload the Logo in JPEG/ JPG/ PNG/ SVG format only.
** RSHAA shall demand for its CDR or other soft copy version.

** The Logo should be designed on a digital platform. The winner of the competition shall be required to submit the design in an editable and open file format. Participants should ensure that original designs are submitted.

** Logo should be designed in color. The size of the logo may vary from 5cm*5cm to 60cm*60cm in either portrait or landscape.

** The Logo should be usable on the website/ social media such as Twitter/ Facebook and on printed materials such as b/w press releases, stationery and signage, labels etc.

** The Logo should be in high resolution with minimum 300 DPI.

Evaluation Criteria

** All the entries received by RSHAA would be scrutinized for awards by a selection Committee.

** Entries would be judged on the basis of elements of creativity, originality, composition, technical excellence, simplicity, artistic merit and visual impact and how well they communicate the theme of MMCSBY.

** The decision of the Selection Committee would be final and binding on all the contestants and no clarifications would be issued to any participants or on any decision of the Selection Committee.

Terms & Conditions

** The prize winning logo(s) may be used by RHSAA for promotional and display purposes at its freewill including but not limited to printing, advertising and any digital platform.

** RSHAA takes no responsibility for corrupted or late entries.
** Employees of RSHAA as well as their family members are not allowed to take part in this Contest.

** Participant(s) is/are required to ensure that his/her profile is accurate and updated since RSHAA would be using this for further communication. This includes details such as name, photo, email id, address and phone number. Entries with incomplete profiles will not be considered.

** The winner will be declared through email or by other way of announcing his/her name.
** There will be no notification to participants of rejected entries.

** Payment to the award winner will be made Any mode. necessary bank details will be taken aft er declaration of the winner(s) of the contest.
