30 Parameter Health Checkup

1. This Voucher entitles you to assured rewards as per the below:
** 1 Free 30 Parameter HCU and
** 1 Free Online Doctor Consultation and
** 1 Month Cult Fit Subscription along with Discount Offers worth upto Rs. 25,000.
2. This offer entitles the holder for a 30-parameter Health Checkup at participating venues in select cities of India.
3. Voucher once booked cannot be rescheduled or cancelled. If the customer fails to show up at the health centre, the voucher will be deemed to be redeemed.
4. The cost of travelling to the health center, SMS and call charges while doing the booking or any other cost associated with availing the offer shall be borne by the participants.
5. A minimum of 48 hours’ prior notice is required to book an appointment. Walk-in customers will not be entertained at the health center.
6. The list of participating health centers may be subject to change.
7. No request for change of date, time, and health center shall be entertained by the Promoter/Agency.
8. In case the successful Participant does not avail the Promo offer on the date and time informed by the Promoter/Agency, the Promoter/Agency shall be relieved of its obligation under the Promo. As such, the successful Participant will have no claim against the Promoter/Agency.
