All you have to do is to take this quick test, answer a few simple questions and make your loved ones’ dreams come true.

Eligibility:
** This Contest is open for all adults upto the age of 50 years resident in Ahmedabad and Bangalore. The person must be an Indian national or a NRI, resident in India for the duration of the Contest (“Participant”). Participation in the Contest is not linked, directly or indirectly, to purchase of any product or service from Max Life.

** To participate in the Contest, the Participant need not be a policyholder of Max Life or pay any fee to Max Life. In order to avoid any conflict of interest, intermediaries of Max Life, employees and directors of Max Life, and Ogilvy & Mather Private Limited, TLG India Private Limited, Ultimedia E-Solutions Private Limited (“Service Providers”) shall not be eligible to participate in this Contest.

Duration:
The Contest duration is from 15th January, 2016 till 15th March, 2016. The dates as mentioned herein are subject to change at the sole discretion of Max Life and a notice of the same will be published on the Microsite. Participants are advised to regularly check the Microsite for details.

Submission Process & Registration Period:
Participants can participate in this Contest by filling up a registration form (“Submission”) and submitting the same through any one of the following media:
** Online via the Microsite;
** Through other digital media (i.e. other websites, where the Contest is publicized);
** Max Life representatives including distributors of Max Life; and
** Any other mode of participation that Max Life decides to add at any stage in the Contest.

Note:
The registration for the Contest will be open with effect from 15th January, 2016 to 15th February, 2016. Submissions received after this period shall not be considered for participation in the Contest. Max Life reserves the right to change the registration period at anytime, at its sole discretion.

The Submission shall be deemed to be incomplete if any of the following mandatory fields contain incorrect details or is left blank:
** Name
** Gender
** Age
** Mobile Number
** Email id
** Salary: Less than 5 lakhs, 5- 10 lakhs, 10 -20 lakhs, greater than 20 lakhs p.a.
** Pincode
** Lifestage: Young & Single, Newly Married, Married with Young kids, Married with older kids, Retired;
** Source from which the Participant received information about the Contest;
** 5 question survey

** An incomplete Submission or a Submission in which the information provided is not factually correct is liable to be rejected by Max Life at its sole and absolute discretion. In addition to the above mentioned fields, some additional fields may be asked for at the time of registration.

** The same may also be requested by Max Life at anytime during the currency of, after the duration of the Contest or at the time of disbursement of prizes, for verification and other purposes. Only one Submission per Participant is allowed. In case of multiple Submissions using same or multiple media by the Participant, only the first completed Submission received by Max Life shall be considered for the Contest and the rest shall be rejected.

** Max Life shall not be responsible in any event whatsoever for the Participant’s inability to participate due to computer malfunction, internet failure, mobile/telephone failure, other technical disruption, and acts of God or otherwise.

Disqualification:
Max Life reserves the right to disqualify any Participant
(i) if it has reasonable grounds to believe that the Participant has breached the terms and conditions of the Contest;
(ii) for acting in a disruptive manner; or acting with the intent to annoy, abuse, threaten or harass any other person, including an employee/director of Max Life
(iii) for providing misleading documentation;
(iv) for tampering with the registration process. Max Life reserves the right to seek damages from any such Participant to the fullest extent permitted by law.

Evaluation:
The entries received will be reviewed by Max Life basis the score received by them on the test portion of the Submission. Submissions with a score of 9.00 shall qualify for participation in a draw of lots. Winners will be determined based on the random selection of such qualifying Submissions. The decision of Max Life based on the random selection shall be final, binding and non-contestable. Max Life shall not entertain any query in this regard.

Prizes:

** The first Submission in each city chosen basis random selection from the qualifying Submissions for that city will receive a Disneyland trip for a family of 4 for 2 Nights/3 Days in Hong Kong (including flight tickets, stay in hotel and 1-day entry ticket to Disneyland, Hong Kong Park) (1st prize).

** The next 10 qualifying Submissions for each city, chosen basis random selection will receive MakeMyTrip vouchers worth INR 5000 (2nd prize). Similarly the subsequent 500 qualifying Submissions for each city, chosen basis random selection will receive BookMyShow vouchers for INR 250 (3rd prize).

** The prizes shall be subject to tax deduction at source, which will be borne by the Participants. All other taxes and levies and incidental costs, which may be levied or incurred on the prizes, shall be fully borne by the successful Participants. If the Participants are not willing to bear the above stated taxes, s/he will forfeit the prize.

Result Intimation:
The final results will published on the Microsite and the same shall also be notified to the winning Participants at their email id and mobile number provided in the Submissions. In the event, the winners cannot be contacted via the contact information available with Max Life, within 2 weeks from the date on which the winners have been announced on the Microsite, the prize will be forfeited and the winner will not be entitled to make any claim thereafter. In case the 1st prize winners do not have a valid passport or is unable to arrange for the same in time for the trip, the prize shall be forfeited.
